It's not uncommon to see that every human being is different, their interests, hobbies, perception, everything has to do with their social community, environment, and other aspects.
But everything has a side to it, positive and negative. And thus, every person has some sort of personality trait which makes them stand out to others.
But when the personality equals a negative vibe, it is termed as a personality disorder. Which tends to destroy relationships of any sort.
There are many types of personality disorders, some come with traumas, some are genetic and can’t be discovered initially, some are from the kind of environment you come from. There are probably more reasons, but these are commonly known.

If a person has a disorder, it's most unlikely that they will sense it right away. They would most likely be aware that something is wrong with them based on the reactions from people they are close with. But people often don’t feel guilty of themselves right away unless it’s proven, which happens when there’s law involved.
Narcissism is a personality disorder with which one of my family members have shown symptoms (although there are only half of the symptoms, not all), it was really important for me to find out because it wasn’t genetic but rather the social circle they were involved with all their lives. They also showed signs of misogyny, lack of empathy towards their family and fragile ego. I can never talk them out of this, not because I don’t have the courage, rather they will not understand (or pretend not to) and put this on a conclusion that I am in the wrong and don't care about the feelings of that person.
You can search for narcissistic personality disorder (NPD), which falls in the category of emotional and impulsive disorders, along with borderline personality disorder (BPD) and antisocial personality disorder (ASPD).
If you are suspicious of yourself or someone who is different from others personality wise, convince yourself to talk to the therapist to clear things out. Mental health should always be a priority for you and your close ones along with physical health. Stay healthy and stay safe.
